Add scores and severity tresholds to a survey

The Scores feature allows you to assign points to answer choices in a questionnaire in order to calculate a result. Displaying an individual or global result enables you to measure and analyze the results of care for a patient or patients along a care trajectory.

Here are the key elements of this feature:

  • Assign points to answer choices: each answer choice (for multiple-choice questions only) in a questionnaire can be assigned a point value, enabling users' answers to be evaluated quantitatively.
  • Use formulas to calculate results:
    • create formulas to calculate results based on multiple questions’ scores.
    • you can also define severity tresholds for each formula to analyze results more easily.
  • Scores display: After completing the survey, a score is calculated and displayed for each patient
    • Individual score: score that is specific to each patient depending on their answers.
    • Average score: this score calculated the average result based on all patients’ answers to a same question or group of questions.

This feature is particularly useful for PROM (Patient-Reported Outcome Measures) surveys, where following individual and global trends are essential to assess a patient’s quality of life or the results of a treatment plan.

How to use scores?

  1. Create a survey
  1. Add multiple-choice questions
  1. In your questions, activate the Scores and assign points to each answer choice
  1. Create formulas that will allow you to analyze patients’ results easily
    1. Define severity tresholds (optional)
  1. Analyze results in the patient record

Add scores to an existing question

Assign points to a question’s answer choices.

  1. Access the list of Surveys through the main navigation.
  1. Click on the Survey you wish to edit.
  1. Click on Edit.
  1. Click on the question you wish to edit.
  1. Go to Scores.
  1. Enable the Scores features by activating the switch.
  1. Assign a unique ID to your question.
  1. Assign points to each answer choice.
  1. Click on Save.
💡
You cannot assign points to a secondary or an other choice.

Scores can only be activated for multiple-choice questions.

Create a formula

Formulas allow you to calculate results based on a patient’s answers.

  1. Access the list of Surveys through the main navigation.
  1. Click on the Survey you wish to edit.
  1. Click on Edit.
  1. Click on Formulas.
  1. Click on Add.
  1. Name your formula and add a short description (optional).
  1. Assign a unique ID to your formula.
  1. Create your formula.
  1. Click on Save.
💡
Scores will be calculated for new completions only.

Add severity tresholds to formulas

Severity tresholds allow you to grade respondants’ answers and read the results more easily.

  1. Access the list of Surveys through the main navigation.
  1. Click on the Survey you wish to edit.
  1. Click on Edit.
  1. Click on Formulas.
  1. In the three-dot menu, click on Edit.
  1. Click on the Severity Tresholds tab.
  1. Activate the severity tresholds feature by flipping the switch on.
  1. Select a severity treshold.
  1. Select an operator.
  1. Enter a numbered value.
  1. If you wish to add several severity tresholds, click on Add a severity treshold and repeat steps 8 to 10.
  1. Click on Save.
💡
You can add a maximum of 5 severity tresholds: Severe, High, Moderate, Low or None.

Edit an answer’s score

  1. Access the list of Surveys through the main navigation.
  1. Click on the Survey you wish to edit.
  1. Click on Edit.
  1. Click on the question you wish to edit.
  1. Click on Scores.
  1. Edit the scores.
  1. Click on Save.

Edit a formula

  1. Click on the Survey you wish to edit.
  1. Click on Edit.
  1. Click on Formulas.
  1. Click on the three-dot menu of the formula you wish to edit.
  1. Click on Edit.
  1. Edit the formula.
  1. Click on Save.

Delete a formula

  1. Access the list of Surveys through the main navigation.
  1. Click on the Survey you wish to edit.
  1. Click on Edit.
  1. Click on Formulas.
  1. Click on the three-dot menu of the formula you wish to edit.
  1. Click on Delete.

  1. Click on the Survey you wish to edit.
  1. Click on Edit.
  1. Click on Formulas.
  1. Click on the three-dot menu of the formula you wish to edit.
  1. Click on Edit.
  1. Click on Severity tresholds.
  1. Edit the tresholds.
  1. Click on Save.

Delete a severity treshold

  1. Click on Edit.
  1. Click on Formulas.

  1. Click on Severity tresholds.
  1. Click on Delete this treshold.
